The GroteApproach Performance Appraisal Form includes five required pages: Organizational
Competencies, Job Competencies, Key Job Responsibilities, Goals/Major Projects, and Major Achievements.
Organizational Competencies
The first page of the form includes a list of the skills, traits, and attributes that leadership expects every member of the organization to demonstrate, regardless of the individual's job or position in the company.
Job Competencies
The second page includes a list of the employee’s job-specific competencies.
Key Job Responsibilities
The third page of the form includes the major components of the employee’s job; the big rocks of the position that would be listed in the individual’s job description.
Goals / Major Projects
The fourth page includes the employee’s developmental and operational goals as well as any special projects assigned to the individual.
Major Achievements
The last page of the form is used to list the employee's top three achievements accomplished during the year.
